Data Clerk: Right to Care – Pretoria

Data Mangement 

Capture and scan all consent and follow-up forms from Right to Care-supported VMMC sites onto various systems.  All original documents must be submitted to the District Office within seven days after completion of the circumcision.

Record all consent forms onto VMMC registers.

Ensure data accuracy by identifying data quality errors and communicate these to the relevant stakeholders.

Submit / escalate daily error reports to the relevant programme staff.

Capture VMMC-related data.

Manage VMMC booking registers.

Ensure that all informed consent forms have been properly signed by clients or their parents / guardians before filing a client file

Quality Assurance

Verify the completeness of the consent-, follow-up forms and patient registers.

Verify the completeness and accuracy of information captured against the source document.  This verification must include both Right to Care and Department of Health data systems.

Manage individual follow-ups of queried source documents within three days of initial query.

Screen all potential clients and ensure that they meet the specified and agreed criteria in order to be circumcised.

Ensure all required documents are available for a client to be circumcised. 

Reporting

Report and submit daily, weekly and monthly statistics from supported VMMC sites as per agreed deadlines, reporting criteria and procedures.  

Compile and submit standardised weekly status reports as per the reporting lines.

In collaboration with the national call centre, ensure that all leads are followed up and scheduled accurately with regular reporting as per national call centre requirements.

Assist in verification of all data before reporting.

Report any policy violation. 

Program Output

Report vehicle incident immediately to the supervisor and complete an incident report

Be flexible to adjust to the transport schedule as needs arise

Provide assistance to all people as per the Batho Pele and Patients Rights Charter principles.

Perform any other relevant and reasonable tasks which may be required from time to time

Ensure all clients tested HIV positive are linked to care and started treatment and record on the register accordingly

Ensure stock and suppliers management/control and timely reporting of any missing or broken to line manager

Obtain informed consent from clients by providing them with essential information as per the National Department of Health (NDoH) and PEPFAR VMMC/HTS guidelines.  

Provide age- and culturally appropriate group and individual counselling to clients within language of their understanding.

Screen patients for Sexually Transmitted Infections (STIs) as per current STI guidelines and Tuberculosis (TB) by asking the four standard TB screening questions and refer patients accordingly.

Ensure that the key elements of VMMC education are included in counselling sessions (i.e. risks, benefits, description of surgery, explanation of partial protective effect of VMMC procedure, VMMC being a permanent procedure, importance of knowing HIV status, and post-operative care).

Conduct pre- and post-HIV counselling and testing as per most recent NDoH HTS guidelines

Ensure all clients tested HIV positive are linked to care and started treatment and record on the register accordingly

Confirm that all linked clients have reached a care and treatment facility within a week of referral.

Follow up on all linked clients at one-, three- and six months into care and treatment.

Verify that all linked clients are still in care and treatment and adhere to their treatment

Ensure sharing of task as when required 

Business Continuity

Perform any other relevant and reasonable tasks which may be required from time to time.  

Fill in for other counsellors at sites as and when required by line management.

Assist in collection and delivering of clients

Familiarise self with rules and regulations of the Health Act and other relevant legislation.

Familiarise self with Right to Care standards, policies and procedures.

Requirements

Matric (Senior Certificate)

Basic Computer Literacy

A valid South African driver’s license

Additional qualifications in Office Administration desirable.

Minimum of one-year clerical experience and basic HIV knowledge.

Two to three years clerical experience and basic VMMC knowledge.
